When I blew out mt C5-6 disc in my neck the inital pain was all upper back/shoulder blade area. Felt like a friggin knife stabbing and twisting along the medial boarder of the scapula and into the rhomboids. The neck pain came a few days later but was not nearly as severe as the upper back pain was.
Some signs that it could be a disc: Shooting pains/numbess or weakness anywhere below the shoulder, into the arm or hand. Also,do you get relief if you put your hands up behind your head? That position will usually release some pressure from the nerve and provide temporary relief if it's a disc. Ice is the way to go until you get some good painkillers. Get some anti-inflammatoies going(Ibuprofen @800mg every 8 hrs) If you find the Vicodin/narcotic does not give you as much relief as it should have based on past experience, that may indicate that it's nerve pain and not a muscle. Neurological pain is much harder to mask with narcotic analgesics than musculoskeletal pain is,the main benifit you'll get if it's a nerve is the sedation buzz effect.
Hope you get a good report from the Doc. Let me know if I can help.

I hope everything gets better. If you have anymore pain killers you know where you can send them.:D

I feel your pain bro'. Just got done having a MRI done on my lower back about an hour ago. Bulged a disc 13 months ago and fucked it up again last Saturday doing straight leg dead lifts. Find out tomorrow if the doc thinks I need surgery or what. It is always something ain't it. It is like Deca said, any numbness or tingling in the limbs means disc. Sounds like you have a real bad muscle pull. I would send you some of my painkillers - but I kind of need them myself. Nothing worse than neck or back pain IMO.

I had a terrible pain like that and had to stop working out for a week. It still took about 3 weeks to feel good. I guess mine was just a pull. I did it using the shrug machine. I dont know how but I must have been doing something wrong. It sucks because you can not hardly sleep so your tired all day and have pain all day.
