I usually change mine up pretty much weekly. I don't go in with a set routine....too many times have had to wait on equipment in past, so I just go with flow. I do try to make sure to include two incline exercises....usually one type of press and one fly movement.
A guy I work with had back surgery and ended up with some nerve damage. He is having the same problems you are. He is the middle of a lawsuit....hope that's not what you're having. Good luck
I always do supersets for my arms. I do one set of biceps immediately followed by one for triceps. I usually do 3-4 sets of each then on to the next superset....usually do about four exercises for each. Doing the same bodypart but going from one exercise to the next, I've always called a giant...
