Why Steroid Users Can’t Donate Blood

By George Touliatos, MD

Q: Why do blood donation centers deny the receiving of blood coming from steroid users, or even HRT patients?

A: Because steroid abuse leads to prolonged bleeding time and impaired coagulation. INR or international normalized ratio,a blood test that indicates how well the blood is able to clot, elevates – meaning the blood clots more slowly than desired. Higher INR equals prolonged bleeding time. Steroids also increase the chances for blood clotting because they elevate fibrinogen. Moreover, both AAS/HRT use will lead to erythrocytosis and this is a problem when it comes to a blood transfusion. Blood viscosity increases and this may lead to ischemic stroke in the patient who receives the blood. Furthermore, if the patient suffers from LHF, his condition can deteriorate. The only possible treatment for symptom relief is therapeutic phlebotomy. Blood will be dropped, but not transfused and wasted afterward.
