Estradiol, Libido and Keeping Hormones in Balance

Q: Does Dr. T agree with Dr. Hertoghe that estradiol should be kept in the range of 20-30 pg/ml, and if out of range should Arimidex be used? I know that Dr. T mentioned using Proviron and that might lower the aromatase activity.





A: Those reference ranges of estrogens are provided in men with optimal use of testosterone, in other words <1000ng/ml. It is important to keep the ratio of T/E2 within a certain range. The problem arises when we have elevated estradiol with a moderate level of testosterone; this is when gynecomastia kicks (of course prolactine, DHT and IGF-1 are also important). Three-digital estradiol with a TT/FT within the range will cause issues in duration of the erection. On the other hand, single-digit E2 can impair sex drive and desire as well. From a personal experience, the best libido comes when estradiol is close to 50ng/ml and this can be managed with moderate use of anastrozole. We have to know that estrogens are considered pre-thrombotic, plus they can contribute to the proliferation of the cancerous cells of the prostate. On the other hand, estrogens are heart protective, improve mood, sexual desire and glycogen synthesis, but also improve androgen receptors’ affinity to the steroid molecules. So there has to be a balance to everything and respect of all hormones.
