sounds like you're getting closer to it... the pause comes at the end of the set. so in other words if you're benching and you're on your heavy set and you get 7 reps, you pause and then get another 3 or 4 reps and then pause again and get another 1 or 2 reps. so you'd write it in your journal something like this:
Bench Press 225 x 7 , 4, 2
