I always replace the needle after sucking up from the vial, and once there was a tiny drop of oil on the top of the syringe, so that an oil film formed between the syringe top and the new needle. I stuck myself, aspirated and pressed down the plunger, but the needle got off by the pressure and the oil film, and there was a nice cover of precious gear all over my leg and the floor.
