Testosterone Replacement: Subcutaneous Injections vs. Intramuscular

By George Touliatos, MD

Q: Is there any benefit of subcutaneous injections of testosterone replacement?

A: Sub C shots have the benefit of being injected into the fat under the skin. In this way, there is no scar tissue creation like when we inject deep into the muscle in the case of an intramuscular (IM) injection. Sub C injections are performed with an insulin needle that is 31 gauge, which is tiny. The disadvantage of subcutaneous injections is the poor blood supply of adipose tissue. This leads to slower absorption of the substance, compared to IM shots. Another speculation is that injected testosterone into the fat will be more likely to aromatize, especially in men who are overweight. However, this theory doesn’t exist. The fact is that testosterone is no longer active until it is separated from the ester (cypionate, enanthate, propionate, undecanoate). Therefore, it’s converting to estrogen after its separation when it is already in the bloodstream. Another disadvantage of the subcutaneous injection is the creation of poly lumps under the skin, especially in lean men with single-digit body fat percentage (six-pack).
